How do I get started?

If you are interested in taking advantage of this opportunity, the fi rst step is to obtain your Partner Code. It may be printed on the back of this card, or see your program administrator.

Once you have your Partner Code,
follow these simple steps:

• Enter your Partner Code, the last four digits of your Social Security Number (Social Insurance Number), name, address, city, state (province) and ZIP (postal) code to obtain your Personal Identification Number (PIN)
